Instructions for facilities looking to submit their plan of correction through the QUAD A portal.
After a survey report is generated for your facility, you will receive an email notifying you that your survey report is ready and directing you to enter your plan(s) of correction online.
Click the “List of Deficiencies” button to review your report and begin crafting your response. You must first log into your account to see the list of deficiencies.
If this is your first time logging into your account, you will need to reset your password. To reset your password, click the “Forgot Password?” link. Type in your email address and click the Send Reset Link” button. You will need to use the email address the survey report email was sent to.
Next, go to your email and open the QUAD A - Forgot Password email to set a new password. Click the “Set a new password” button. You will be redirected to the reset password page. Simply enter the desired password and confirm the password. Please note that passwords are case sensitive.
You will then be able to log in to the portal and view your deficiencies list. Once you log in, you will be directed to your application page, which will display any open applications you are associated with. You may have more than one application available if you are representing more than one facility.
To navigate to the deficiencies list click on the notifications icon, shaped like a bell, in the upper right hand corner. Locate the correct notification based on the facility’s ID and click view.
At any time, you can access your facility’s statement of deficiency list by clicking the “Survey” tab in the top navigation. This will take you to the survey module, which will list all facilities that you are associated with. Click on the desired facility. This will show you the reports for surveys that have been conducted for the selected facility.
Click on the desired survey to view the survey details including the “Corrections Due” date under the Reporting Information section. From the left-hand menu click on “Statement of Deficiency.” This screen will show you the list of deficiency citations for the survey. Click on a Standard Number to view the statement of deficiency. For each deficiency you'll be able to view the survey findings in order to successfully submit a plan of correction.
To submit a plan of correction you will need to provide the date that you plan to complete the correction, the name of the facility administrator or director, and the name and title of the party responsible for overseeing the corrective action.
In addition, you will need to detail the Actions to be taken and a description of the steps that will be taken to correct the issue. The Procedure for Implementation will describe how the actions to be taken will be implemented, who will be involved in the actions, and the ways in which the proposed action will satisfy the cited deficiency.
In the Monitoring and Tracking section you will need to list the name and title of the person who is responsible for the monitoring, who will monitor this to ensure future compliance, the frequency of monitoring activities and decision-making meetings, and the frequency and length of monitoring.
You will also need to provide the types of tools that will be used to track compliance, for example, score sheets or review forms, how the facility will document ongoing monitoring, documentation of an accepted baseline and the threshold for assessing acceptable compliance. Note, the threshold for compliance should be a percentage. Address whether or not any noncompliance be allowed. Describe the actions to be taken if compliance is not achieved and if noncompliance is found, the steps that will be taken.
Be sure to hit the “Save” button. Repeat the steps above for each deficiency on your deficiencies list. Once you have saved a plan of corrective action for every deficiency on the list click the blue “Submit” button in the lower right-hand corner. Once you click this button you will be asked to confirm that you would like to submit the plans of correction to QUAD A for review. Click the “Yes” button.
Plans of correction that are deemed not acceptable will be returned to you for revision. If a plan of correction is returned to you, you will receive another notice to follow the steps as above. Once you have accessed each deficiency, you will see blue comment bubbles to the right of the text box where QUAD A staff has provided direction.
Click on the comment bubble to review the feedback provided by QUAD A. You can respond to comments directly, but please note this does not revise the plan of corrective action. This provides an opportunity for dialog between the facility and QUAD A. A new plan of correction may be needed for a deficiency if the previous one is declined. To make this process more efficient the data from the first plan of correction will be replicated so you can easily edit them.
